Please download, read through, and fill out the forms and return them to us as soon as possible so we can be prepared for your particular needs and goals. Please email completed forms to info@abundanthealthwellnescenter.org at your earliest convenience. Once we receive all the forms we will set up a pre-retreat consultation. This step is crucial for you to get the most benefit out of your health retreat.

Given the personalized nature of our health retreats and limited size of participants we accept per retreat, we do not offer refunds. However, if you need to cancel your reservation and if we are able to fill your spot, then we will gladly apply your investment toward a future retreat within the next 12-month period. Please note that you will be responsible for any difference in price at the time of rebooking. If we cannot fill your spot we will not be able to give you a credit toward a future retreat.
